(9) Sore Throat (Painful, scratchy, or difficulty swallowing)

(10) Runny or Stuffy/Congested Nose (Not related to allergies)

Results of Screening Questions

- If you answer NO to all questions 1 through 5, you have passed and your dancer may attend dance in-studio today.

-if you answer YES to any question #1 through 5 or checked a symptom in question #7, you have not passed screening and you cannot attend dance in-studio today. We must advise you to self-isolate at home and contact a health care provider to get advice or an assessment, including if a COVID-19 test is required.

- If any of the answers to these screening questions change during the day, this screening result is no longer valid and you will need to screen again, wherever necessary.

RETURNING TO DANCE IN-STUDIO

- If you have selected YES to any Question #1 to 5 or checked any of the symptoms in Question #5 your dancer and their siblings must isolate and not attend dance class in-studio for the following amount of time:

* If Fully Vaccinated or 11 & Under - dancers must isolate for 5 days prior to returning to dance and must be symptom free for 24-hours prior to returning to dance.

* If partially vaccinated, immunocompromised, or over 12 and unvaccinated - dancers must isolate for 10 days prior to returning to dance and must be symptom free for 24-hours prior to returning to dance.

*All siblings must also isolate if someone is feeling unwell in their household even if they have no symptoms and feel fine.
