This form is to acknowledge an employee or other vendor that went above and beyond their role for any reason. This could include but is not limited to an employee performing tasks outside of his/her normal responsibility to ensure the success of an event, or another vendor (e.g. photographer or videographer) helping in any way which they are not required (e.g. taking photos/video for our use). We want to make sure that employees are rewarded for exceptional behavior and any vendors are recognized and properly thanked, ensuring a great relationship going forward.
