License & Permit Document Request Form

This form is for current GHS students who need documentation to obtain a learners permit or real ID. Once this form has been completed, the following documents will be sent via email:
● Official school transcript for current year
● Official letter from school (proof of enrollment)
Please be advised that it could take up to 10 days for documents to be sent.
