CONSENT and CODE OF CONDUCT
Kids’ Night Out! (herein referred to as KNO!) seeks to provide a welcoming environment where all participants can be safe and have a good time playing, interacting, learning and growing.
The KNO! team members are expected to:
Respect all children and parents/guardians
Provide a safe environment for all children
Strive to reflect Jesus’ ways and values
Children participating in KNO! are expected to:
Respect leaders (by listening attentively and following instructions)
Respect peers (by treating others as they would like to be treated, by participating in activities safely)
Respect property (by using all equipment and supplies properly and by taking good care of the facilities)
Children’s behavior that does not meet the above expectations will be addressed:
First occurrence: child is reminded of expectations.
Second occurrence: child is asked to refrain from participation for a specified period of time and rejoins later.
Third occurrence: child is asked to refrain for the remainder of the evening.
Persistent misbehavior patterns (several times in one evening, several evenings with repeated occurrences, or any other similar patterns that disrupt, interrupt or otherwise create an unpleasant environment for others) will result in suspension from KNO! The duration of such suspension will be at the discretion of the KNO! coordinator.
Parents/guardians will be notified at all stages and are urged to work with their children to be respectful.
I am the parent/guardian of the child(ren) named in the KNO! registration form, and with my signature I give my child(ren) permission to participate in the KNO! activities.
By sending my child(ren) to KNO!, I attest that my child(ren) is/are NOT sick or experiencing any symptoms of illnesses that can be contagious or harmful, including, but not limited to, cough, rash, hives, vomiting, diarrhea, and fever.
I recognize that the possibility exists for injury in the course of KNO! activities. I understand that simple first aid may be rendered as necessary. In the event of a medical emergency, I give permission to the KNO! team and the First Baptist Church of Flushing to have my child(ren) treated at the nearest medical facility with the understanding that all transportation costs and medical expenses are solely my responsibility. I agree to hold harmless First Baptist Church of Flushing and its officers, employees, agents, and representatives from all causes of action, liabilities and claims and related costs that I or my child(ren) may have against any of them arising from any and all bases in connection with the participation in the KNO! program.
I also consent to photographs and/or videos of my child(ren) to be taken during KNO! activities and for said photographs and/or videos to be published for promotional purposes at the discretion of First Baptist Church of Flushing.
This permission expires one year after the date it is signed.
