CPFTA is now accepting applications for the Class of 2026. The deadline for submitting is Wednesday, May 1, 2024.
• 2.0 grade point average by the end of Sophomore year
• Good attendance (90% and above is recommended)
• Complete an online application found at CPFTA.com
• Complete a program consent form and send it to CPFTA@CPS.EDU (the link is
Class of 2026 Consent Form)
• Hold a valid school ID, state ID
or driver’s license
• Provide 2 letters of recommendations (use the following form
Class of 2026 Letter of Recommendation Form• Have a current physical on file at your CPS or Charter school or, if you are attending a private or home school, you are able to present proof of a current physical examination in September of 2024
• Complete a background check
Application ProcessPart 1 - Complete the Online Application All applicants have until Wednesday, May 1, 2024, to complete the online portion of the CPFTA application. As part of the online application, applicants are required to share the completed Program Consent Form. The consent form should be emailed to

Class of 2026 Consent Form Candidates will be notified via email if you are eligible to move to part 2 of the CPFTA Application. This email will be sent between April and May of 2024. Processing will take place in late May and is required for all applicants of CPFTA.
Part 2- Background checkOnce Part 1 of a student’s CPFTA online application has been accepted, he/she will be notified to report to the Chicago Police Department Education to complete Part 2 of the CPFTA application. This will take place in May of 2024.
Upon arrival at the appointed location, students must turn in the following information to the CPFTA staff. The forms include;
1. Personal History Questionnaire (will be sent to students via email before processing)
2. Background investigation form (will be sent to students via email before processing)
3. Authorization for Release of Personal Information form (will be sent to students via email before processing)
4. 2 Letters of Recommendation (one from a school official and one from a community leader). The link to the recommendation form is
Class of 2026 Letter of Recommendation Form5. 2 Copies of student’s School ID, Driver’s License or State Identification Card
6. Copy of High School Transcript for G.P.A. This can be an unofficial copy.
Students will be notified after processing regarding acceptance into the program. (usually in June)
