Please complete a separate registration form for each junior. Money will not be refunded for missed days and missed days are not guaranteed to be made up. Programs will take place rain or shine unless unsafe. If a junior has to cancel after registration has taken place and prior to the program starting a cancellation fee will apply. In the result of injury or accident sustained to my junior, I release all liability to Humber Valley Resort, its staff & any others helping out with the junior programs. I know that golf is a high speed sport. Any injury or damage, caused by my junior to other golfers, staff or property, or to my junior, is my responsibility. In case of injury or accident, if I am not able to be contacted, I give permission to Humber Valley Resort or its staff to obtain proper medical attention during my absence. Humber Valley Resort will not be responsible for the loss of any personal property, however caused.

Humber Valley Resort has the right to take & use golf photos to endorse any & all golf programs as they see fit in any form of print, web or other media to promote junior golf. I have read, understand and agree to the terms and conditions set out above. This form must be signed/checked in order to complete registration.
