Parent/Guardian Waiver : I hereby give my consent and permission for my youth named above to participate in the Madison High School's Girls Volleyball Camp. I recognize and acknowledge that there are certain risks of physical injury with participation, and I agree to assume the full risk of any injuries, damages or losses, which my youth may sustain, or cause, as a result of participating in any and all activities connected with or associated with the camp. I hereby waive, relinquish and fully release and discharge La Follette High School, the Madison Metropolitan School District and their/its officers, agents and employees from any and all claims which I or my youth may have or which may arise for injuries, damages or losses I or my youth sustain, or which I or my youth may cause during participation in the camp. I agree to indemnify and hold harmless and defend the La Follette High School, and the Madison Metropolitan School District and their/its officers, agents and employees from all claims resulting from injuries, damages and losses sustained during and arising out of, the activities of the camp and/or fitness activity. I have read and fully understand the above Consent, Release and Hold Harmless Agreement.
