Health/Medical Conditions *
Do you have any health conditions that could impair you and/or be aggravated by this sport? If yes, please describe them and how best to help you in case of emergency. Please keep in mind, outrigger canoe paddling involves not only physical activity, but also being on water away from shore for stretches of time, so also share any health conditions (e.g. bee sting allergy) that we should be aware of in that regard.

Cold Water Safety Guidelines *
In the spring and fall, the water in Cayuga Lake can be cold, which requires attention with regard to clothing and safety. Please acknowledge that you have read our cold water safety guidelines: https://cayugaoutrigger.com/cold-water-policy/
Required
Huli Drill *
Huli (hoo-lee) is the Hawaiian word for flip over and is most commonly used in Hawai'i to describe BBQ chicken cooked on a spit. For paddlers, huli drills are an essential part of being safe and water-wise, in case a sudden swell or error causes the canoe to huli unexpectedly. We ask that everyone review our webpage about hulis and recovery: https://cayugaoutrigger.com/paddlers-handbook/huli-and-recovery/ and watch the linked video. All members are required to participate in a huli drill at some time during the season! If you have any questions, ask!

Member Covid-19 Policies/Protocols
CDC guidelines no longer mandate masks for outdoor activities. They suggest considerate caution about sustained, close contact between people who are not part of the same household, particularly in strenuous exercise.

Cayuga Outrigger Canoe Club members and guest will follow these policies:

1) We are guests of the Merrill Sailing Center and will follow their guidelines for masks and social distance.
a) outside, club members are not required to mask, but should use their discretion about masking and distance, particularly with regard to the comfort of others, especially people who are not fully vaccinated.
b) when inside the Sailing Center, members should observe/follow the most current Cornell mask guidelines.
2) For OC6s, all paddlers must be fully vaccinated, including if they have had covid – since we use a Cornell facility, we use its definition for"fully vaccinated": that you have had all doses in the primary series of an FDA- or WHO-authorized or approved COVID-19 vaccine (e.g., two doses of Pfizer or Moderna, or a single dose of Johnson & Johnson).
a) paddlers should provide proof of vaccination to a board member (or coach) prior to their first practice. The club will maintain a list of eligible paddlers (who verified their vaccination), but keep details about verification private.
b) people who are unable to be vaccinated for medical reasons should speak to a board member.
3) Masks are optional on the water.
4) Members should remember not to share food or drink with one another except those who are in their same household/pod.
5) Paddlers with covid symptoms should not attend practice. Common symptoms include: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea.

Covid-19 Risk Informed Consent
I understand that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. I further understand that COVID-19 is contagious and is believed to spread by person-to-person contact; and, as a result, federal and state health agencies recommend social distancing.

I recognize that the Cayuga Outrigger Canoe Club, its board members, and coaches are monitoring this situation and have put in place reasonable preventative measures aimed to reduce the spread of COVID-19. I hereby acknowledge and assume the risk of becoming infected with COVID-19 through my participation in club activities on and off water.

I understand that, if I have a COVID-19 infection, and even if I do not have any symptoms for the same, proceeding with club activities can lead to a higher chance of complication and spread of the infection to those around me, including family and friends.

I understand that possible exposure to COVID-19 may result in the following: a positive COVID-19 diagnosis, extended quarantine/self-isolation, additional tests, and I may need additional care that may require me to go to an emergency room or a hospital.

I understand all the potential risks, including but not limited to the potential short-term and long-term complications related to COVID-19, and I would like to proceed with participation in athletic competition.
