Welcome to another year of Faith Formation at Assumption! Our program here is unique, since our parish partners with families to foster 'domestic churches' where the faith flourishes at home. We offer three different models - Home Based, Study Hall, and Classroom - to fit the needs of our families, and place the kids in the right track according to where they are in their formation journey. This form will help you place your child into the right model and track for them. Please register one child at a time. You'll receive a prompt at the end to submit a new registration if needed.
