TMSD Parent Selection Form: 20-21 School Year

The Tussey Mountain School District is planning to welcome students and staff back to school in August. The district has developed a Reopening Plan which includes health and safety guidelines, mandates from the state Department of Health and CDC, and includes operating models that align with each of the governor’s three COVID-19 phases (Red, Yellow and Green) with the ultimate goal of resuming in-person teaching and learning for the entire school year. The district is asking parents to review the TMSD Reopening Plan, available at www.tmsd.net and complete the following form regarding your selection for the upcoming school year. Please note that details of the Reopening Plan are subject to change based on guidelines from local, state, and federal authorities.

Please complete one form for each student that is enrolled at TMSD for the upcoming school year. For planning purposes, please submit this form by August 5, 2020.
