PTS Forms will ONLY be sent to you once they are made available to us (usually within the next week or two). The form will come via the DocHub program - fill in the areas you are prompted to and then it will be sent to the VP Travel automatically to complete. Once the VP Travel completes the form, it will be emailed back to you. It is YOUR responsibility to print the form and bring it to your tryout.
