Relationship Between Parental Needs, Child’s Quality of Life, and Occupational Therapy Services During Cancer Treatment
You are being asked to participate in a research study.  Before you give your consent to volunteer, it is important that you read the following information and ask as many questions as necessary to be sure that you understand what you will be asked to do.

Investigators: The investigators involved in the study are graduate students of California State University, Dominguez Hills (CSUDH), specifically the Masters of Occupational Therapy Program. This research group is made up of three graduate students: Jina Song, Jihoo Seo, and Emily Shleppey. The students are under the supervision of the principal investigator, Yan-hua Huang,  PhD OTR/L. She is a professor in the MSOT Program. She can be contacted through this number: 310-243-3626.

Purpose of the Study and Participant Inclusion Criteria: The purpose of this study is to explore the relationship between the perceived needs of parents of children diagnosed with cancer, their child’s quality of life, and whether or not their child received occupational therapy services during cancer treatment. The following is the participant inclusion criteria:
   ► Between 18-64 years of age
   ► Parent of a child diagnosed with cancer
   ► Child is diagnosed between the ages 0 and 18 years old
   ► Child is currently receiving cancer treatment
   ► Can read English

Description of the Study: The study will be completed through an online survey. The survey comprises of 72 questions asking about: 1) demographic information, 2) child’s diagnosis and treatment, 3) parental needs, and 4) child’s quality of life. The survey may take 10-15 minutes.

Risks or Discomforts: Because of the personal and possible emotional nature of some of the questions that may be included in the study, you may feel uncomfortable to continue answering the questions. If at any time you feel like you would like to discontinue your participation in the study you may do so. Choosing to withdraw from the study as a whole will not affect your relationship with the researchers or the school.

Benefits of the Study: There are no guaranteed direct benefits to you, the participant in the study, but the findings of the study may benefit individuals who have children diagnosed with cancer in the future. This information can also be of value to practitioners working with this population. Our study can also inspire further research interest regarding parent care in the realm of oncology and occupational therapy.

Confidentiality: Throughout the study, data will be kept confidential; only those directly involved, the researchers, will have access to the information. Files will be password protected to ensure confidentiality and security. The survey is anonymous and the only information collected that can be used to identify participants are general demographic information. All data will be destroyed upon completion of the study.

Incentives to Participate: There are no incentives for participation in this study.

Voluntary Nature of Participation: Participation in this study is voluntary.  Your choice of whether or not to participate will not influence your future relations with California State University, Dominguez Hills or the investigators.  If you decide to participate, you are free to withdraw your consent and to stop your participation at any time without penalty or loss of benefits to which you are entitled.
