Winfield Public Library Summer Reading 2024 Registration
This is the official online registration form for Winfield Public Library's 2024 Summer Reading Program. Please read the form and answer carefully. You will need to complete a registration form for EACH CHILD who will be attending. Visit the library and follow us on Facebook for more information on our summer reading program. Due to limited space, children who are registered will be given priority to attend programs. We ask that parents and guardians please consider limiting adult attendance to one adult per household to allow more children to attend the programs. Summer Reading live programs are for children ages 4-12. We have additional programming during the summer for children ages 0-3.

Waiver of Liability and Hold Harmless Agreement: In consideration for receiving permission to participate in the Winfield Public Library Summer Reading Program 2024, I hereby release, waive, discharge and covenant not to sue the City of Winfield, Winfield Public Library, agents, servants, performers or employees from any and all liability claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, my child, persons whom I am responsible or any of the property belonging to me  while participating in such activity, or while in or on the premises where the activity is being conducted. The library is a public place.  Anyone may enter and use the facilities.  Children left alone in the building can be vulnerable and at risk. The Winfield Public Library does not assume responsibility for any children.  Staff cannot and do not take over parental or childcare responsibilities for children who come into the library. *
PARENTS' RESPONSIBILITY: Library policy states a parent, guardian, or responsible caregiver, 16 or older, must accompany children ages 10 and younger while visiting all areas of the library and library grounds; however, we will be allowing parents to drop off and pick up their children during summer reading due to limited space. Children will be signed in by a parent/guardian and signed out by the same parent/guardian at the end of the program. Children MUST be picked up within 45 minutes from the time the program begins. Parents/guardians of children who demonstrate behavioral issues will be asked to stay with the child at all summer reading programs. CHILDREN'S RESPONSIBILITY: Children, like all library users, are required to respect library property and to act in a manner appropriate to the use and function of the library.  Children are subject to the same Rules of Conduct as other patrons and the same consequences, including being asked to leave the library.  A parent or capable caregiver must accompany children of any age with mental, physical, or emotional disabilities, which affect behavior or decision-making skills or make supervision necessary. *
TO PROTECT UNATTENDED CHILDREN: Parents, guardians, and designated adults are required to be familiar with the library’s hours and to make advance arrangements to pick up or otherwise transport their children before the library closes.  A minor left unattended at closing time may be deemed at risk.  Every reasonable effort will be made by the staff to assist the child in contacting an appropriate adult.  If no responsible adult is reached, or the child is not picked up before time of library closing, library personnel may notify the police to assume custody of the child.  Staff will not transport children home or to any other destination under any circumstances.  If a child is left unattended at the library in circumstances that give library staff cause for concern about the child’s safety or welfare, staff may contact child protective or law enforcement authorities. *
The library never uses personal names just images. I hereby consent to the collection and use of my child's personal images by photography or video recording.  I acknowledge these may be used on the Winfield Public Library’s website, Facebook and Instagram pages, in newsletters and publications as well as distributed to members. I further acknowledge that my image may be used to promote Summer Reading in the future.  I give this consent voluntarily. *
