Please remember:

-We unfortunately do not have control over off service schedules. If you want a particular holiday off and you are off-service, you will need to send that request to the off service chiefs.
- If you are on anesthesia/tox/vacation/elective, this will likely count as your holiday off.
- Deadline for submitting the request form will be September 5th. Please get it in as soon as possible.
