Greetings Faith Formation Families,

The planning of the Faith Formation program for the 2021 - 2022 school year has begun. This year we will be using a full regular schedule with all grades in attendance.

To help update our information on students, and contact information you are asked to please complete the linked registration form for EACH CHILD who will be attending Faith Formation this year. Please include an/all information that may have changed in the past year. This will help greatly to keep us in contact with you should the need arise.
