You are kindly invited to participate in a research study on Coronavirus disease 2019 (COVID-19) vaccine. We are interested in learning more about what you would think about a hypothetical coronavirus vaccine. If you agree to be part of the research study, you will be asked to complete a survey. This will take less than 10 minutes to complete.

Benefits of Participating
Filling the questionnaire will give you the opportunity to express your views and for your voice to be heard about COVID-19 vaccine. Others will also benefit from the knowledge gained from this study while the government and public health experts and institutions will be duly informed.

Risks and discomforts
We will protect the confidentiality of your research records by uploading the data to a password-protected folder. Your name and any other information that can identify you will not be collected as part of the survey.

Compensation
You will not receive compensation from the researchers. Participating in this study is completely voluntary.  Even if you decide to participate now, you may change your mind and stop at any time.  You may choose not to answer any survey question, or continue with the interview for any reason.

Information collected in this project may be shared with other researchers and for purpose of academic publication but we will not share any information that could identify you.
