Transcript Release for Students 17 years of age and younger (Parent Permission Required)

By completing this form, I, the parent or legal guardian, of the following student, under the age of 18, am giving permission for Kiski Area High School to release an official or unofficial copy of my student’s transcript. *Students 18 years of age must request their own transcript using the "Transcript Release for Students 18 years of age and older."

Reasons to release a transcript may include, but are not limited to:
● Student HAS APPLIED to Colleges/Universities/Trade or Vocational Schools
● Requested/required for a scholarship opportunity
● Requested/required for a job shadow, work release, or field trip
● Student or parent requests a copy

Please note: Kiski Area High school WILL NOT release a student’s transcript to any military recruiter or military organization without additional written consent from the student’s parent/guardian.
