  • Legal Documentation is required; this can be emailed to mnewell@maplerun.org, faxed to 802-527-6467, or dropped off to the Guidance Office during regular school hours. 
  • Parent/Guardian signature is required and must have legal rights to make changes for you. 
  • To make any future changes, you will need to submit a new request.
  • Please email the BFA Registrar with any questions about this form to mnewell@maplerun.org
  • Please allow 24-48 hours for processing, once form has been submitted. You will receive a confirmation when this is updated in PowerSchool.
