Platform Chinook's Puppy Application

If you are interested in a puppy or dog from us, either now or in the future, please fill out the questionnaire below.

Please keep in mind that we breed specifically for versatile dogs who can excel in many venues. Breeding Prospects, Conformation ring, and all the fun dog sports! We focus on creating healthy dogs that are confident and outgoing, suitable for most everyone.

We do not put anyone on the wait list, or approve anyone for a puppy, without a completed questionnaire. Please include a timeline for when you'd like to ideally add a new puppy.

If we do not respond right away- please be patient. We are a busy family with young children, dog events, full time jobs, and these wonderful dogs who require training, daily exercise, attention, and care. Give us a day or two and if you have not received a reply, send us an email. I promise we are not ignoring you! Thank you for your kindness and understanding :)

Your puppy will be crate trained when you receive him, will you continue to crate him in your home? If you are not going to crate your puppy how will you keep him out of trouble while you are not available to supervise him?

Where will the puppy stay when you are not at home. Explain.
Where will the puppy sleep at night?

How much interaction do you foresee between yourself and your Chinook on a day to day basis?  How active a life style do you have?
