All players will be registered with the Ontario Basketball Association (OBA) as Oxford Attack Basketball Club House League Players.

WAIVER: I, understand that any athlete who does not abide by the rules and requirements to participate in the Oxford Attack Basketball house league are subject to dismissal without reimbursement or recourse.  I fully relieve any participant of the Oxford Attack Basketball Club house league any responsibility pertaining to loss or damage to the athlete`s personal property or injuries sustained by the athlete (including, without limitation, any medical or dental expenses incurred because of his/her participation at this house league. I further assume full responsibility for any damage caused by the athlete to facility premises.  The athlete is in good health and able to participate in the physical activities of our vigorous program.  In case of sudden onset of injury or illness, I hereby authorize the Oxford Attack Basketball Club house league personnel to take immediate action according to their best judgement.
