West OK District Tournament: Belonging and Inclusion Incident Report Form 2024
This form is for the purpose of reporting incidents regarding discrimination, harassment, etc. at the NSDA West OK District Tournament. We are committed to addressing any incidents that occur at the tournament. Beyond filling out this form, you may contact Sara Nichols directly at sara.nichols@edmondschools.net. She will be getting in contact with you ASAP regarding next steps and ways to possibly resolve the incident.

Any person who believes that they have experienced behavior in violation of the National Speech & Debate Association Harassment and Discrimination Policy or relevant tenets of the Code of Honor or Coaches Code of Ethics may complete this form to report the violation.
After completing the form, the person who believes they have a valid concern should talk with the Belonging and Inclusion Advocate (Sara Nichols), who will collect information from involved parties and recommend a corrective course of action.
Belonging and Inclusion Advocates will keep the information disclosed and confidential as possible. Ethically, Belonging and Inclusion Advocates are required to take appropriate action and notify authorities if students engage in behavior that presents clear and imminent danger to themselves or others, in the case of known or suspected child abuse or neglect, or in the case of known or suspected sexual violence.
