The SCIPHC is happy to try to accommodate this request; but, to better assist you and your needs, we ask that you fill out this form thoroughly and return it to us within 2 days of receiving it. Your request will then be reviewed, and we will contact you as soon as possible.
This request form must be submitted at least 60 days before the date of the requested event. Any request form submitted in less than 60 days prior to the even may not be considered for approval.
