By completing this form, we understand that (MUST SELECT ALL IF CHALLENGING): *

The student must pass the exam with a minimum score of 80% in order to earn credits and will be issued an A” or “B” grade accordingly.

I understand that if the student does not earn a minimum score of 80% on the challenge exam, he/she must enroll in and successfully complete the names course (if required for graduation).

This student has permission to challenge the course indicated above.

The challenge exam will be a department-developed exam administered by the department chair or designee.

The student cannot later enroll in the same course to improve a challenge grade. The challenge grade is permanent on the student’s academic record.

A student may challenge a particular course only once (1).
