If you wish for your child to participate in Didcot Phoenix Cycling Club (DPCC) events, please read the following information on both pages, complete the form below and sign the parental consent notice.

Any information provided about your child will be securely and confidentially stored, processed and destroyed by DPCC in accordance with Data privacy policy. It will only be used for the purpose of contacting you or your child regarding future DPCC events, clubs and associations where your child could get involved in cycling; or to provide you with information about DPCC membership.

With your permission DPCC may also take photographs/video footage during club activities. These images may be used in coaching resources, placed on the DPCC website or social media, or for general publicity purposes.

Please note that helmets are compulsory for children at DPCC events.
