VCHC Annual Membership Form
Use this form if you are paying your VCHC institutional or individual membership dues using PayPal or a credit card. If using a check, download and print the PDF form posted on the membership website and mail it with your check.

VCHC membership is available to institutional or individuals upon payment of annual dues. 

 Annual dues for institutions are $70.00 per calendar year, due in January.

Annual dues for individual members are $10.00 per calendar year.

Individual membership dues are included in the spring meeting registration fee. Individual membership should only be paid for those who are not attending the spring conference.

Both institutional and individual membership dues must be paid in order to vote in VCHC business. Institutional members are allowed one voting representative at business meetings. Please specify the voting representative below.
