Dear Parent or Guardian,
Our fall sports preseasons begin in August! HS-August 8, MS August 15
Please fill out one registration for each athlete.
*An up-to-date physical form is required for each athlete to participate.
   -All athletic forms can be found at schaumburgchristian.com/athletic-forms
   -Forms can be dropped off at the school office or emailed to the health office

This helps cover coaching, transportation, referee expenses, tournaments, and equipment expenses. Athletes selecting more than one sport should contact Coach McVey for details. *
