Due to the COVID-19 pandemic, our government has mandated that a limited number of people may gather in enclosed public and community spaces. To comply with the law, New Hope Missionary Baptist Church (New Hope) has restricted in-person attendance at worship services to no more than 100 people in the building at any time. We need your help to ensure that we do not exceed that number on Sunday morning.

Please use this questionnaire to inform New Hope that you and/or members of your household intend to attend worship services in-person on Sunday morning. Your individual or household registration must be received by Friday of each week to ensure a spot is held for you and/or your household. Attendees that have COVID-19 symptoms or who fail to register by the weekly Friday deadline SHOULD NOT attend in-person worship services. However, alternative methods of participation via Facebook Live, conference call, or recording will be available to all members and guests.

For the safety of others, we strongly advise you to stay home and avoid all public or community spaces until you have no symptoms. COVID-19 is highly contagious and is known to spread mainly from person-to-person contact. By attending New Hope Missionary Baptist Church, you agree to abide by the procedures established by the church to protect attendees and staff, and you voluntarily assume the risk that you and/or your family may be exposed to or infected by COVID-19 either at the church, in the church worship service, or at other church events. You agree to assume all the risks of attendance and participation for you and your family, and you waive any liability against the church, its subsidiaries, and/or other managed entities.
