*PFC Extreme will make every attempt to form individual Boys and Girls teams in each Age Group. In the event that PFC cannot offer enough spots to form these teams, the genders could be combined to form Boys teams with girls participating on them.

The player listed above has permission to participate in the PFC Extreme select team tryouts. I do hereby assume all risks in connection with the participation in said activities and release and discharge injuries to person and property that may befall the herein named player while participating in such activities. PFC Extreme has permission to photograph the player listed for the sole purpose of identification as required for MOSSL carding purposes. Further, I agree to furnish upon acceptance of an offer to play, a valid birth certificate for the player listed above and an initial team fee deposit to reserve a spot on the roster of the team for which they were offered a spot.

Players are trying out for teams formed for the upcoming Fall season and next year's Spring season. Players who accept an offer to play for PFC Extreme will be required to make the PFC Extreme team their primary team for ONE FULL YEAR (Fall & Spring). PFC Extreme teams play in the Mid Ohio Select Soccer League (MOSSL) as governed by the Ohio South Youth Soccer Association.
