每個比賽日 (11月5日、11月6日及11月13日) 都必須分別填寫一份健康申報表格。
Everyone MUST fill in this health form for each game day.

所收集的資料僅為確保會場參加者的安全及健康，並於活動後31日後銷毀。
Information collected will only be used to ensure the safety and health of the participants in the event venue, and will be destroyed after 31 days of the event.

參加者如同時參加個人項目，而會帶不同觀眾入場的情況，已登記的觀眾可於比賽當天填寫由本會提供的健康申報表進場。
If participant enrolled in individual event, and coming with different spectators, registered spectator can sumbit the hard copy health form in the event date for entry.
