DOGSAMUST Dog Adoption Application
This adoption application is for DOGSAMUST dogs only. I understand that if I do not contact my veterinarian and allow DOGSAMUST to speak with him/her about me, or if I do not fill this application out in a complete and detailed manner, this application will not be processed. The adoption fee is $250 which includes spay/neuter, vaccinations, basic vet care, and 8 to 10 weeks of basic obedience training. I understand that I am responsible for the care of this dog. I am responsible for any medical care that might be necessary once the dog is in my possession. I will provide annual vaccinations and heartworm preventative.   *
I will provide the dog with a collar and name tag with my phone number in case he/she gets lost. *
I will not keep this dog crated over 6 hours at any time. I understand that crating an animal over 6 hours can cause this dog to be highly stressed and may in turn result in behavioral problems. *
I will not keep this dog on a chain or tied up in any fashion nor will I allow the dog to ride loose in the back of a pick-up truck. *
I will not use this dog in any sort of dog fighting or as a bait dog/used to train fighting dogs. *
I agree to ONLY use POSITIVE reinforcement to train this dog and will not at ANY TIME hit, strike, spank, kick, or use ANY physical means to discipline this dog. I will contact DOGSAMUST immediately with any behavioral issues regarding the dog. I understand that using my voice loudly and saying “NO” or using a spray bottle of water is an appropriate way to discipline, when IMMEDIATE correction is needed to train my dog. I understand that my dog will not understand any correction other than IMMEDIATE correction DURING inappropriate behavior training. *
I understand that DOGSAMUST will not be held liable for any damages that this dog might cause including harm to a person or damages to property. *
All dogs are tested for heartworms before entering the program and are given monthly heartworm prevention for the entirety of their time in the prison. We do our best to ensure the health and safety of every dog and puppy adopted out. However, some dogs in the program have been rescued from deplorable conditions. As it takes at least 6-7 months from the time of the initial infection for heartworm disease to show up on a test, it is beyond DOGAMUST's ability to know if a dog might test positive for heartworms several months after adoption. I acknowledge that I, as the adopter, assume all responsibility for any and all treatment necessary to treat heartworms if my dog tests positive after adoption. *
I agree that before euthanizing this animal for any reason, I will contact DOGSAMUST and give them the option to reclaim the animal prior to euthanizing. *
