This is the 2024-25 registration form for FRESHMEN in MARCHING BAND/PERCUSSION/COLOR GUARD. This form encompasses the registration form, medical consent and permission to travel form.  In order to complete this form, you will need to have your health insurance card information (with group number and phone number) and your child's physician's name and phone number.  You will also need the names of any medications your child takes on a regular basis.  Now you are ready to begin!
