LIFE League STL Participant Release Form

By completing this registration form, I, the undersigned parent/legal guardian of the minor participant, hereby provide my consent to LIFE League STL and its representatives to:

Use of Likeness: Capture and use photographs, video recordings, and any other forms of my child’s likeness for use in promotional materials, social media posts, and other publications as deemed appropriate by LIFE League STL without seeking further approval or offering compensation.

Emergency Medical Treatment: Provide emergency medical treatment to my child in the case of injury, accident, or illness during participation in LIFE League STL activities. This includes, but is not limited to, transportation to a medical facility, hospitalization, and treatment as deemed necessary by qualified medical personnel.

Liability Waiver: I understand that participation in sports and related activities carries inherent risks of injuries. I hereby release, waive, discharge, and covenant not to sue LIFE League STL, its officers, coaches, volunteers, agents, and representatives from any and all liabilities, claims, demands, or injuries, including death, that may be sustained by my child while participating in such activities.

Acknowledgment of Responsibility: I acknowledge that it is my responsibility to ensure my child’s fitness for the program and to equip my child with all necessary gear and protections. I agree to inform LIFE League STL of any medical or health conditions that may affect the participant’s safe involvement.

By submitting this registration form, I affirm that I have read, fully understand, and agree to the terms stated herein. I recognize that this release is a legally binding agreement and signify my agreement by checking the box provided in this form.
