Fill out this form to request that your child is not photographed for district purposes.

This is not for annual school pictures and yearbook. This form only applies to photography and video for district use such as social media, newsletters, district website and news releases.

Note: This form is for photographs by district personnel only. It does not cover photos that may be taken by other parents/guardians at events that are open to the public.

Requests must be put in each new school year.

NOTE: If you are filing out this form after Sept. 2022, you MUST email the Communications Specialist at mcavoyminork@sgfcsd.org to inform them of the change.
