We are so excited your kid(s) are joining us for this year's STELLAR VBS, it's going to be out of this world! First, please give us some information about yourself, the parent/guardian. You will need to fill out this form once per child.

This year we will have kid size VBS shirts and a worship CD of all the VBS songs available for purchase at $10.00 each. There is a separate order form on our church website and church app to fill out if you would like to reserve and purchase them now. They will be available for pick-up at our VBS registration area during VBS. You may also purchase them in person during the week of VBS.  *
Does your child have any food allergies? If none, say none. If yes, what are they and what are the symptoms and the level of severity of the symptoms that we should watch for? If they have an epi-pen please let us know.  *
Our snacks for the week are:
Monday - "Chewy" granola bars (various flavors)
Tuesday - pretzel rings 
Wednesday - apple slices 
Thursday - clementine oranges
Friday - rice crisp chips 
Sunday  - hamburger/hotdog potluck 

If your child cannot have a snack, please bring your own snack in a ziplock bag with their name clearly printed on it on the days she or he can't eat that snack. Please turn the snack in at the registration table to be saved for them for snack time.

We understand that life is busy, and while we hope that you can join us for every day of VBS, it would help us in our preparations if you can let us know ahead of time if you know certain days that your child cannot attend. Please only check the boxes for the days that you will NOT be attending.  *

Below are our activity participation agreements, indicating that you (parent/guardian) give your child permission to participate in Vacation Bible School, sponsored and hosted by Sandpoint Church of the Nazarene (aka the "activity sponsor") located on the premises of Sandpoint Church of the Nazarene 477954 US-95, Ponderay, ID, from June 19-23, and 25, 2023. 
Every day at VBS we make a short video of all the fun we had and we send the link to the video out to all the VBS families and our church to enjoy. We also take a group photo of each crew of kids so they can have a keepsake of the week. In addition, our Family VBS Sunday Showcase is recorded and made available on our church YouTube page. If you do not want your child to appear in the daily videos or group photo please indicate that below. Children who opt-out will receive a sticker-dot on their nametag as a visual reminder to our leaders that they are not to be included in photos or videos during the week, and children are advised to sit with his or her family behind camera on Family VBS Sunday Showcase if they do not want to be in that recording. *
I acknowledge that participation in the activity described above involves risk to the participant (and to the participant's parents or guardians, if the participant is a minor), and may result in various types of injury, including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. 

In consideration for the opportunity to participate in the activity described above (VBS 2023), the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation and transportation to and from the activity. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by the sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to the "activity sponsor"). Further, the participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the activity sponsor for any injury arising directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the activity sponsor, the participant, or otherwise. 

If a dispute over this agreement or any claim arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.
