INSTRUCTIONS — PLEASE READ CAREFULLY

IMPORTANT: This form is for math courses with conflicting final exam times. Students with confirmed schedule conflicts will be issued an alternate exam time and location, NOT a different exam date.

Students with three scheduled final exams in one day should refer to the NJIT Final Exam Conflict Policy (http://www.njit.edu/registrar/exams/conflict.php) to determine which exam is to be rescheduled.

IMPORTANT: Non-academic examination conflicts will NOT be approved (e.g. planned trip or interview, work schedule, special event, requesting more time to study, etc.).

This application must be submitted no later than Monday, April 28, 2024.

In order to have your application processed, you must complete the following steps:

1) Provide the course name and section number of each course that conflicts with your final exam time.

2) Complete and submit this application.

3) You will be contacted at a later date via mathpermits@njit.edu with further instructions to resolve your Final Exam Conflict.

4) DO NOT contact the Math Department to inquire about the status of your application until one week prior to final exam week.

5) Please DO NOT submit multiple applications as repeated requests for updates will only delay the processing of your application.

6) Please be advised, the DMS Office arranges testing accommodations ONLY when provided timely notification from the Office of Accessibility Resources and Services (OARS). Students in need of accommodations should contact OARS as soon as possible. (https://www.njit.edu/studentsuccess/accessibility)
