All youth members participating in Youth Cricket sessions & matches are required to complete and submit this membership from.
This form is designed to be completed by the parent, or legal guardian of the player.
Players aged 12+ years are also asked to provide their own consent for video/photography.
Please ensure your child (if 12yrs+) has provided consent before you complete this form on their behalf.
