Nutrition & Wellness Study
WELCOME

            Welcome and thank you for your interest in this study. This survey is exploring the relationships between diet and mental health. If you are 18 years old or older you are allowed to join this study. The principal investigator for the study is Alison T. Fletcher. She is a doctoral student in the Department of Clinical Psychology at Antioch University New England, Keene, New Hampshire. Alison is conducting this study to further her understanding in the field under the supervision of her research advisor, Dr. Karen Meteyer.

Purpose of the Project:

This study is looking at the relationships between Nutrition, Exercise, Mindfulness, Cognitive Function, Quality of Life, Stress, Anxiety, and Depression.

Procedures:

First you will be asked to answer demographic questions. Next you will be asked questions about your eating habits, then questions about how you perceive your quality of life, experiences of anxiety, depression, stress, mindfulness, general cognitive function, and your physical activity habits. The survey will take about 30 minutes.

Benefits of Participation:

Possible benefit of being in this study is becoming aware of your eating habits, and perception of stress, anxiety and depression. This may cause you to make changes in these areas. Although not everyone will have this benefit. The goal of the study is to improve the understanding of how people experience symptoms of anxiety, depression, and stress and the relationship between these factors and diet.  

Risks:

The risks of participating in this study are expected to be minimal. The questions in the survey are not likely to cause more discomfort than experienced in daily life. 

 

Confidentiality and Anonymity:

Identifying information will NOT be collected (IP address, email address, name or phone number). All records of this study will be kept private. Only the primary researcher will have access to the survey data. Your answers will be kept anonymous. The data will be stored on a password protected computer owned by the principal researcher. Any report of this study will not identify any persons.

Refusal/Withdrawal:

Participation in this survey is VOLUNTARY. If you choose to be in the study, you may stop at any time without penalties. Simply exit the survey.

Debriefing:

No formal debriefing will be given as the study does not anticipate any negative consequences. However, if you have any questions about the study, please contact Alison at afletcher@antioch.edu
