Thank you for taking the time to complete this survey which we've designed to help us understand your transition process, and to get your thoughts and feelings about how it went.

If you're not sure, the word 'Transition' refers to the process of moving from children's services to an adult care team. We'd ask you to complete this survey once your own transition is finished so that we can get the full picture of how it worked for you. It should take no more than half an hour.

If you're not a patient of an Inherited Metabolic Disorder, or have not yet completed transition, please don't complete this survey but feel free to get in touch using the contact details below to discuss how you might get involved in our research.

This survey will ask you some practical and some emotional questions but please only tell us what you're comfortable with, and remember that you don't have to complete the survey if you don't want to. Finally, if you're telling us about a particular person in your answers please do not give their name.
