SCMA Leadership Distinction Award Application

Purpose: Recognize members of the SCMA for their involvement and achievements in organized medicine and advocacy. Involvement and participation in these activities will also create a natural pipeline for professional advancement.

Deadline: Applications are due annually on April 1. Awards will be presented at the SCMA Annual Meeting. Submit your application to info@scmedical.org with the subject line “Leadership Distinction Award.”

You must complete 10 (or more) of the activities listed below in order to be eligible for the award. Please indicate which activities you completed by supplying the date completed. If you did not complete a particular event, please leave the date information blank.
