Please use this form to check out resource material for your children.  Each subject is listed on a separate page.  You'll need to complete one form for each student (library program constraints).

Due to finite supplies, we ask that only ONE BOX PER SUBJECT PER STUDENT is checked out at a time.  

Please return material at the END OF TWO WEEKS. 

*We expect games and resource supplies to get used. Loosing a die or card is within expected usage ranges.  If a number of pieces in a game or other similar item, please add a note to your box/bag when it gets returned as this will save us restocking time.

Note: Curriculum are ordered through a different form and are kept for the entire school year (or semester for semester based courses)
