Minor Library Card Application or Card Renewal
This form is for prospective library patrons between the ages of 4 and 17.  

Online applications will be processed by our staff within two business days.  You will receive an email when the library card account is created or renewed.  If you need a card or renewal sooner than this, please come to the library in-person for immediate assistance. 

A Parent/Guardian of the Minor will need to show us their driver's license or state ID (to verify address) to complete this application.  You can email a scan/photo of the front AND back of your ID to milancirc@gmail.com or show it to library staff when you come to pick up your child's card. 

A child/teen can receive a free card from our library if they:
- live in our service area (Milan, MICHIGAN or the contract areas listed below) 
- are currently enrolled in the Milan Area Schools system
- would like to use MILibraryCard privileges at our library (using a card from another MiLibraryCard participating library)
