There will NOT be an in person Registration this year.

**YOU WILL NEED EACH PLAYER'S USA HOCKEY REGISTRATION NUMBER TO COMPLETE THIS FORM.**

Via email, we will:
1. Update you on Youth Hockey information.
2. Coordinate to collect fees (not paid via Paypal or mailed).
3. Learn about DIBS/Sign-up for Volunteer/Hockey Hut shifts - updates will be provided.
4. Provide information about how we plan to Issue your Cash Calendar tickets.

Fees
The following are the MEYHA Registration fees:
$25 Termites
$25 Mini-Mites (Popsicles)
$95 Mites
$105 Squirts/10U
$115 Peewees/12U
$125 Bantams
**There is a $50 City of Hoyt Lakes for the arena and recreation fees. The association will pay of this fee.** This fee was doubled this year, so the MEYHA will continue to pay $25.
(NOTE: Registration fees go toward the cost of ice time for practices, games, and tournaments, referees for home games and tournaments, a jersey or jerseys to use for the season.)
(Additional equipment is available for all levels --Termites & Mini-Mites/Pops are free, Mites and above pay a rental fee.)
Additional Requirements (ALL Levels: Termites-Bantams)
In addition to the fees for Registration, we ask families to contribute in the following ways, each explained below.
1. Volunteer Fee ($150)  (NOTE: Mites-Bantams ONLY)
2. Cash Calendar Fundraiser ($300)  (NOTE: Mites-Bantams ONLY)
3. Hockey Hut Fee ($100)  (NOTE: Mini-Mites("Pops")-Bantams)

1. Volunteer Fee (Levels: Mites-Bantams)
Each family will include $150 with their Registration fee.  This fee is paid back to you (via check from the Treasurer) upon completion of 20 volunteer hours.
Volunteer hours are earned in the following ways:
~Home Games (Announcer, Books, Clock, Penalty Box)
~4-on-4 Hockey Hut shifts
~Tournaments (Running the tourney games with the above jobs, Hockey Hut hours are consider "Volunteer Hours" during tournaments, Raffle table (if running a raffle))
~Team Managers
Other opportunities are available, too, as needs arise throughout the season.
ALL Volunteer Hours are tracked through the use of DIBS on the website.
You MUST register on the www.meyha.org site and claim your DIBS items (each job is listed).  First come, first serve.
Once your 20 hours are completed, you contact the treasurer to let him/her know your hours are done and he/she will cut you a check, reimbursing the $150.

Volunteer Fee *
I understand the $150 Volunteer Fee is cashed and then reimbursed, upon my family's completion of 20 Volunteer Hours.  I further understand that it is my responibility to claim the hours on DIBS, track my hours, and contact the Treasurer for reimbursement of my $150 when my hours are completed.
2. Cash Calendar Fundraiser (Levels: Mites-Bantams)
Each family will receive 15 Cash Calendar tickets to sell, for $20 each.
The Association collects this money up front, a total of $300 per family (not per player), since the prizes are cash.
Reimburse yourself as you sell the tickets.

Cash Calendar Fundraiser *
I understand that I will be paying $300 and will receive 15 tickets to sell on behalf of the MEYHA for $20 each.  I will return the tickets by the deadline required for the drawing.
3. Hockey Hut Fee (ALL Levels: Termites - Bantams)
Each family will include $100 with their Registration fee ON A SEPARATE CHECK.  This fee is either HELD or CASHED, depending on your choice of OPTION 1 or OPTION 2, below:

**NEW - REDUCED NUMBER OF SHIFTS, with the need to cover less shifts, we are able to decrease the number of shifts this year, from 6 to 2!**

Option 1: Families are asked to cover 2 shifts, which can be completed during 2 Youth or High School Games in the Hockey Hut during the Hockey Hut Season (Sept 1st-May 1st).  This is approximately 5-6 hours.  (Separate from the 20 Volunteer Hours.)  These shifts are tracked in DIBS.  Claim your shifts on a first come, first serve basis.
The $100 check is HELD if you choose this option.  This original check is either returned to you or destroyed upon completion of the 2 shifts.  Contact the Treasurer once you have completed your shifts.

Option 2: Families are asked to cover 2 shifts via "buyout".
The $100 check is immediately cashed and your Hockey Hut shifts will be covered by someone else (possibly an individual or group the association may need to hire to cover the requirements of our lease with the City of Hoyt Lakes).
