This form must be completed to inform Kiwi Cabins Custom Builds Ltd of your authorisation and agreement regarding contactless delivery at New Zealand's Covid-19 Alert Level 3.

Please note if you do not agree to our terms of contactless collection we withhold the right to make a judgement call to cancel the order based on the safety of our team.

I will maintain social distancing of 2m minimum from the team when I am collecting a product from collection location as organised. *

I will adhere to the laws and regulations surrounding level 3 and wear a face mask/covering to collection. *

I will adhere to the laws and regulations surrounding contact tracing at level 3. I will scan in using the Covid-19 QR scanner or physically sign in on pen and paper provided. *

I understand these precautions are in place for the safety of the Kiwi Cabins staff and myself. *
