Here is the form to fill out before our first session together. Everything shared in this form is 100% confidential and only shared with Leanna, owner of Huntress Coaching. If the form is not completed, this will result in losing your session slot. Please take time to answer to the best of your ability.. and always put the answer that comes to your mind first... even if it doesn't make sense or you aren't sure if it ever really happened that way.

I am very excited and humbled you have decided to dive into understanding that your relationship to food is deeper than your willpower! I hope this can bring you some much needed peace.. that you are enough right now and always have been to create healing with your past and allow you to step into your life as the version of yourself you have always envisioned!

Does food ever feel like protection against not allowing yourself to truly "feel" or share your emotions? If so, what emotions do you think you are trying to protect? *
What is the downside of you losing weight?  Examples: Is the extra weight providing protection? Does it feel safe to carrying extra weight? *
What is the upside of being overweight?  Examples: Does it keep attention off you? Does it mean you don't have to show up a certain way? Does it lower others' expectations of you? *
If you weren't spending so much energy on your weight, what would your life be like? *
What was your relationship like with your dad or the dominant male figure that raised you? How did you respond to him growing up? *
What was your relationship like with your mom or the main female figure that raised you? How did you respond to her growing up? *
Have you experienced any sexual abuse? Sexual abuse can even be any "sexual" phrases and/or conversations said to you that impacted you negatively... also others criticizing your body in any way or making you feel uncomfortable about your body. *
