Thank you for contacting the Center for Faculty Excellence. This form allows you to request support from the staff at the CFE on the use of technology resources. It is our goal to help you utilize these tools in your role as faculty. 

1) If you are experiencing technical support issues like your camera is not functioning, a sympodium or PC is not working, or your Google Account password does not work, these are issues for the Help Desk at the Office of Technology. They can be reached at 985-549-5555.

2) If you have a specific question about utilizing the new Canvas LMS, please first consult the Canvas Guides and/or Canvas Tier I support available 24/7. If they cannot assist you, you will then be directed to the appropriate department related to your question.

3) If you have a specific LMS request (described below), then please use the specific form linked. If not, you may use this form for general support requests.

LMS REQUEST FORMS:
Sandbox Request Form: A sandbox provides you with a space to design and develop your course in Canvas before moving that content into the actual course each semester.
Student Access Request Form: This form can be used by faculty to request continued access to their course after the course would normally be hidden from students at the close of the semester. This is for those students who have received an "incomplete" and have been given extended time to finish the coursework.
Faculty Access Request Form: Use this form to request special access for faculty members. When making your request, please be sure to choose the role the faculty member should be given in the course. Only one request can be made per submission.
