Parent/Guardian S.A.P. Referral Form
Fort Cherry School District has a Student Assistance Program (SAP) for grades K–12 that is designed to offer support and services to students experiencing academic, behavioral, and/or emotional difficulties that may pose barriers to school success. Any staff member and/or parent/guardian can refer students to SAP.  

The SAP Team, consisting of teachers, counselor(s), parent(s), and principal, will meet to discuss concerns and will suggest ways to help your child reach his/her full potential. With your assistance, a plan will be created to assist your child to be more successful in school.
