EHS Off Campus Lunch Application
Please complete the following form to apply for Off Campus lunch privileges for the 2020-2021 school year.

Off Campus eligibility requirements:
     -Current 11th or 12 grade student
     -No discipline issues
     -No attendance issues

Off Campus expectations:
     -Students will maintain mature and responsible conduct
     -Students will maintain respect for the property and personal rights of others and the community
     -Students will arrive back on campus in time for class
     -Students will not transport or socialize with other students off campus who do not have Off Campus
      permissions

If approved for Off Campus lunch privileges, you will be notified by email and/or phone.
