This form reserves your team’s place in the KSUIF’s 2019 Trick or Treat Film Festival. Note that equipment is first come first serve and this form will not guarantee you SRL equipment. The registration fee of $40 per team and this form are due in the KSUIF office by Monday, October 21st. The fees and form may be turned in separately. The fee is non-refundable.

An email will be sent to your captain’s email after we receive this form. Please choose your team captain wisely as they will be responsible for all correspondence between KSUIF and your team.
