All merchant applications must be paid in full and on my desk by December 13, 2019

1. To apply please completely fill out the following merchant application online accurately

2. IF YOU ARE PAYING BY MAIL: Mail a check or money order for the merchant fees, including your helper fees, for the total amount. Partial payments are not accepted. Mail in payment must be postmarked by December 6, 2019. Checks are made payable to SCA INC. Fill out this form, mark paying by check/money order  and hit send. You may print out your copy and mail it to the address listed below, or just print section 6 and mail it with payment.

3. IF YOU ARE PAYING BY PAYPAL: you must have your FULL payment in by Dec 13th 2019.Paying by Paypal
Complete the PayPal Merchant Pre-registration Form
Please allow up to 72 hours for the invoice.
You will receive a PayPal invoice
PayPal invoice will be cancelled 72 hours after it has been sent, if it is not paid.
After Paying the Paypal invoice you will receive a receipt

4. Email us a copy of your Arizona Tax License, apply in advance apply at
https://azdor.gov/transaction-privilege-tax/tpt-license/applying-tpt-license

5. (Food Merchants Only) Email or mail a copy of your Maricopa County Special Food Permit.

6. (Food Merchants Only) Email a complete menu of the items you wish to sell at Estrella War

7. Proof of SCA Membership for the booth owner and all family and staff. A $5.00 Non-Member Registration fee (NMR) is added for each non-member.

8. You Can email a  signed Consent to Participate and Release from Liability Release Form for you and
everyone in your party including children. Or bring it to the event.

9. A Minor’s Consent to Participate and Hold Harmless Agreement for any minor children
that you are not the legal guardian of. (NOT optional) Non-custodial adults must also have legal documents to bring a minor to Estrella. Please see http://estrellawar.org/wp-content/uploads/2013/08/sca-minors-consent-to-participate-and-hold-harmless-agreement.pdf

10. If you bounce a check your application will be cancelled. Checks will not be cashed
immediately upon arrival. If you think this may be an issue, please send in a Money order or Cashier’s Check.
