My child, named above in this form, has permission to participate in the Junior Personal Improvement Visionary Award Program from August 15-31, 2022. I understand that this activity does NOT involve any medical or psychological treatment.

Conduct During Activity: I understand that my child’s success in the program is based on my child's participation. My child can stop participating at any time. I acknowledge that I have spoken to my child about my child’s need to comply with specific rules, requirements, expectations, and requests for this activity, and that my child should conduct herself with respect towards the mentor, staff, the program itself, AND herself. 

Acknowledgement of Personal Liability and Waiver: I also understand that this program may expose my child to some risks, and I assume any such risk that may arise. I accept full responsibility for all medical expenses for any injuries that might occur to my child by reason of his/her participation. By signing this form, however, I hereby release Prime Pageants(TM), its agents, volunteers, assignees and staff from and against any and all claims, demands, actions, complaint’s, suits and/or all other forms of liability that any of them may sustain (a) arising out of my child’s failure to comply with participation requirements and the participation thereforth or arising out of any damage or injury caused by my child in relation to this program. I also agree to indemnify and hold harmless the released parties from the released claims, including any and all related costs, attorney fees, liabilities, settlements, and/or judgements. By typing my name below I give consent to the terms above. I confirm that I have carefully read this Consent and Release and agree to its terms knowingly and voluntarily. I also confirm that I am the parent or legal guardian of the child mentioned above and consent to my child's participation.
